<h2 class="sectHeading">How to apply</h2></span>
<p>Download the Application for Issue or Transfer of Competence Certificate: Funeral Undertaker, which you can find in the document downloads below.</p><p>Complete the form as fully as possible, ensuring that you include as attachments:</p><ul><li>A description of the premises and its location. </li><li>A complete ground-plan of the proposed construction of, or existing, buildings on a scale 1:100.</li><li>A block plan of the surrounding buildings, who occupies them and what they are used for.</li><li>The particulars of any person other than the owner and employees who will be preparing corpses on the premises.</li></ul>
