<h2 class="sectHeading">About<br></h2><p>The City's EAP and Wellness Branch encourages all our employees to live a healthier lifestyle by attending to their mental, social, physical and health needs.<br></p><p>The team offers a range of services including clinical assessments, disease screening and prevention initiatives and holds a number of wellness days and physical well-being events during the year.<br><br>Service providers who are interested in partnering with the EAP and Wellness should register their details on the City’s Wellness Database.</p> <span> <div class="notification with-heading dark-copy pink bg-light-grey"><div class="graphic with-border"> <i class="info note"></i> </div><div class="desc"><h4>Please note</h4><p>You must be willing to offer services at no cost to the City.</p></div></div></span><span> <h2 class="sectHeading">How to register</h2> <p> <strong>Step 1: </strong>Email your details to <a href="mailto:wellness.events@capetown.gov.za" target="_blank">wellness.events@capetown.gov.za</a></p> <p> <strong>Please include the following information in your email:</strong> </p> <ul><li>company name<br></li><li>company registration number</li><li>income tax number</li><li>physical and postal address</li><li>VAT number</li><li>up to five business commodities and a description of each</li></ul> <p> <strong>Step 2: </strong>Once your email has been sent, you will receive an automated reply to confirm that your registration details have been received.</p> <p> <strong>Step 3:</strong> A representative of the EAP and Wellness will contact you when a suitable opportunity arises.</p> <span> <div class="notification with-heading dark-copy pink bg-light-grey"><div class="graphic with-border"> <i class="info remember"></i> </div><div class="desc"><h4>Remember</h4><ul><li>All service providers will need to sign a service level agreement. </li><li>The time frame of the partnership is open, and will be subject to a three-month notice period for termination for non-compliance.</li></ul></div></div></span> <h2 class="sectHeading">Contact us</h2> <p> <em>Telephone:<br><a>021 444 4886 </a><br><br>Email:<br><em><a href="mailto:wellness.events@capetown.gov.za">wellness.events@capetown.gov.za</a></em> </em></p></span>
