Apply for a disability disc

The City issues temporary and permanent disabled parking permits. Temporary permits are valid for six months, and permanent permits are valid for two years. Disabled parking disc can be linked to more than one vehicle. Anyone who qualifies can apply, but the form can only be completed by an occupational health practitioner.

Categories of disabled parking discs

There are three categories of disabled parking discs:

How to apply

Step 1: Download and complete the Disability Disc Application Form from the document downloads section below.

Step 2: Get an Occupational Heath therapist to complete the application form. The occupational health practitioner must hold a qualification in occupational health recognised by the Health Professions Council of South Africa.

The occupational health practitioner is required to confirm that:

  • The applicant’s mobility is severely impaired by an ongoing physical condition.
  • The applicant’s mobility is temporarily or permanently, and severely, impaired.

Please note

We have to confirm that the applicant’s mobility is severely impaired. Only original signed application forms are accepted – copies are not valid.

Step 3: Take your signed application form with all necessary documents (outlined in the box below) and submit them at any of our driver's licence testing centres.

With your completed application form, please also bring:

  • acceptable identification (official ID or driver’s licence)
  • application fee (check the tariff sheet below for more information)
  • two recent identical photographs of the applicant.
