How to register
Step 1: Complete the application form
Step 2: Submit the completed application form:
Step 3: The City will contact the applicant when a registered occupational therapist is ready to assess the applicant and make the necessary travel arrangements.
Step 4: The occupational therapist will assess the applicant, complete the necessary assessment form and submit it to the City.
Step 5: If the applicant is successful his/her information will be entered into the database and a registration number issued. New registered passengers are informed of their registration number and the conditions of carriage. This number must be quoted when they access the service.
If the applicant was unsuccessful the City will inform him/her accordingly.
